Kidney Transplant

Kidney Transplant
When someone has chronic kidney failure or end-stage kidney disease, a kidney transplant can offer them a second chance at life. This life-saving procedure involves placing a healthy kidney from a donor in those who have end stage kidney disease. A successful kidney transplant can help patients live a longer, healthier, and more active life.

As a Paediatric Nephrologist in Pune, I, Dr. Punit Chhajed, work with children and young adults who are facing kidney-related challenges. I understand that the thought of kidney transplant—especially in a child—can be overwhelming for parents. That’s why I believe in explaining things in simple terms so that families can make informed and confident decisions.

What is a Kidney Transplant?

The kidneys are two bean-shaped organs located just under your rib cage. They filter waste and excess fluids from your blood, which then leave the body through urine. When the kidneys fail, they lose their ability to clean the blood properly.

A kidney transplant is a surgery where a healthy kidney from a living or deceased donor is placed into someone whose kidneys no longer function well. People need only one donated kidney to live a healthy life.

Who Needs a Kidney Transplant?

Kidney transplants are usually recommended for people with:

  • End-stage kidney disease (ESRD) or kidney failure
  • Chronic kidney diseases that have gradually worsened over time
  • Severe kidney damage due to congenital conditions, infections, or genetic issues

For children, kidney problems can be caused by birth defects, urinary tract problems, or inherited conditions. A transplant offers them a chance to grow normally, go to school, and enjoy life without the restrictions of dialysis.

Why Choose Kidney Transplant Over Dialysis?

Dialysis is another treatment option for kidney failure, but it only does a part of what healthy kidneys do. While dialysis can help remove waste and fluid, it comes with limitations:

  • Requires frequent hospital visits or home setups
  • Can cause fatigue and dietary restrictions
  • Not a permanent solution

A kidney transplant, on the other hand:

  • Improves quality of life
  • Increases lifespan (especially in younger patients)
  • Allows for more freedom in daily activities
  • Offers better growth and development in children

Types of Kidney Donors:- There are two main types of kidney donors:

1. Living Donor:- A healthy person (usually a close family member like a parent, sibling, or relative) donates one of their kidneys. Humans can live with just one kidney, so a living donor can lead a healthy life after donation.

Advantages of living donor transplant:

  • Better match and success rates
  • Shorter waiting time
  • Planned surgery date

2. Deceased Donor:- A kidney is taken from someone who has recently passed away and had agreed to donate their organs. These kidneys are allocated through national& regional transplant programs based on priority and matching.

The Kidney Transplant Process – Step by Step

  1. Evaluation:- Before a transplant, the patient undergoes several tests to make sure they are healthy enough for surgery and to find a suitable donor match. These tests include blood tests, imaging scans, and heart checks.

  2. Finding a Donor:-If a living donor is available, compatibility tests are done. If not, the patient is placed on the deceased donor transplant waiting list.

  3. The Surgery:- The surgery usually takes about 3–4 hours. The donor kidney is placed in the lower abdomen and connected to nearby blood vessels and the bladder. The old kidneys are usually left in place unless causing problems.

  4. Recovery:- After surgery, the patient is monitored closely. Most children can start eating and walking within a few days and go home in about a week.

  5. Medications and Follow-Up:-  After a transplant, patients need to take immunosuppressant medications for life to prevent the body from rejecting the new kidney. Regular follow-ups and blood tests are crucial.

Kidney Transplant in Children – Special Considerations

As a Paediatric Nephrology Specialist, my focus is on the unique needs of children:

  • Children may need transplants at a young age due to birth defects or rare kidney disorders.
  • The surgical approach, medication dosage, and post-care are all adjusted for growing bodies.
  • Emotional support and counselling are essential for both children and parents during this journey.
  • A child may need more than one kidney transplant in their entire life span

The goal is not just survival, but a happy, healthy childhood with school, sports, and play.

Risks and Success Rates

Like any surgery, kidney transplants come with some risks such as infection, bleeding, or rejection. But with modern techniques, experienced transplant teams, and lifelong follow-up, the success rate is very high.

  • Over 90% of kidney transplants from living donors work well after 1 year
  • Children with successful transplants usually lead a near-normal life

The key to long-term success lies in regular follow-ups, taking medications properly, and maintaining a healthy lifestyle.

Life After a Kidney Transplant

After the recovery phase, most patients can return to:

  • Regular school or work life
  • Playing sports (with doctor’s advice)
  • Eating a more relaxed and balanced diet
  • Traveling and participating in social activities

It’s important to continue seeing your nephrologist regularly, take medicines on time, and protect yourself from infections.
